The Office of Vital Statistics offers certified birth certificates for persons born from 1930 to the present.
To apply for a birth certificate:
- You must be a parent listed on the certificate, a legal representative, a legal guardian (with court appointed papers), or the individual listed on the certificate.
- You must be at least 18 years of age, and must have state issued identification.
Certificates are available for the State of Florida only. Applications for other states may be obtained from our office.
Death certificates date back to 1985 for Lake County only. Death certificates are public records and anyone may obtain certified copies for a fee. If you are not an immediate family member or a legal representative, you will not be able to purchase death certificates listing the cause of death without proper authorization.

For your convenience we accept cash, money orders, Visa, MasterCard, Discover and Debit Cards. Please note Visa and MasterCard are only accepted when you are present in our office, not through the mail.
We do not accept personal checks.
