The Lake County Health Department’s Community Health Improvement Plan now available online
Eustis – The Lake County Health Department’s Community Health Improvement Plan (CHIP), a combined effort of Well Florida Council, community partners including public, private, non-profit and other organizations and residents, is now available online by visiting www.lakechd.com and clicking on the resources menu and choosing the 2012 MAPP Needs Assessment link.
The community health needs assessment process began last year and an initial needs assessment report was provided to the community. The health needs assessment report formed the basis for the development of the Community Health Improvement Plan (CHIP). The CHIP is a guide for community members and agency leaders to help identify the agencies and parties responsible for implementing policies and programs that will influence the health issues most important to
